CNS impairment – Nursing assessments and considerations

CNS impairment could be associated with confusion, delirium, drowsiness, and hallucinations. Confusion is also a condition association with

(1) Hypoglycemia
(2) Ventricular tachycardia
(3) Head injury
(4) Stroke
(5) Volume deficit (i.e., fluid volume imbalance).

Delirium is

(1) Also a symptom of withdrawal in case of barbiturates,
(2) Also a symptom of abuse in case of hallucinogens.

Hallucinations is also a symptom of abuse in case of hallucinogens.

In case of CNS impairment, nurses may consider safety measures for the prevention of injury, orient to surroundings, and avoid activities requiring alertness as, for example, driving a car.
